Please note: Not all COVID-19 related quarantines or absences due to work, school, or daycare closures qualify for coverage. If your absence is not due to a personal or family-related health condition, please contact your Employer prior to submitting your claim/leave with Unum.

Whatever your type of claim, you can file it online. This includes claims for disability, life, accident, critical illness, hospital, dental and vision — plus claims for leave of absence (disability, maternity, FMLA) and wellness benefits for screening tests.
