Update dependent children coverage information

If your Unum policies cover dependent children, submit updated information to remove coverage when they no longer have eligible dependents. Dependents may no longer be eligible for coverage once they reach a certain age—depending on policy provisions. Unum recommends that when employees remove dependents from medical or dental coverage, employers contact their employees to discuss whether any dependents remain eligible.

For group customers, when an employee's youngest child is no longer age eligible, adjust the employee's premium payroll deductions accordingly.

  • List bill customers should make plan changes online or fax per instructions on your paper bill.
  • Self-accounting customers should report number of lives, volume and premium changes on your next bill.

For voluntary benefits customers, please remind your employees to contact Unum if they no longer have eligible dependents due to their age. To remove coverage for dependents, employees should complete the customer service form. If your employees have questions about the form or need clarification regarding child eligibility, please have them contact Unum at 1-800-635-5597.
