INTRODUCTION

Purpose of the Privacy Notice

This Privacy Notice describes Unum Group's practices regarding the collection and use of the personal information of the users of this mobile device software application ("Application"). The purpose of this Application is to provide mobile access to Unum's online services.

Unum Group, through its insuring subsidiaries, provides insurance products and related services, as well as leave administration services. This Privacy Notice only applies to personal information collected in connection with downloading, registering or using this Application, and does not apply to information collected or provided through other means.

In this Privacy Notice, "you," "your," "yours," and their derivations are used to refer to you, the Application user. "We," "us," and "our," and their derivations, and "Unum," are used to refer to Unum Group and its insuring subsidiaries.

Your Consent and Choice

By downloading and using this Application, you are consenting to our collection, use and sharing of your personal information, as described in this Privacy Notice. If you do not agree with the practices described in this Privacy Notice, do not download, register with or use this Application.

If you do install the Application, you may opt-out of all further information collection via the Application by uninstalling the Application. Consult your device manual for instructions on how to uninstall the Application.

This Application is intended to be used in the U.S. If you use this Application while outside the U.S., your information will be transferred to the U.S., and processed and stored under U.S. privacy standards. By using the Application and providing information to us, you consent to such transfer to, and processing in, the U.S.

Privacy Notice Effective Date and Changes

This Privacy Notice is effective as of the date stated above. We may update it from time to time. Your continued use of this Application after we make changes is deemed to be acceptance of those changes. We will not make material changes to how we treat our users' personal information without providing notice, such as by sending an email to you or through other ways, prior to the change becoming effective.

THE INFORMATION WE COLLECT

"Personal information" means individually identifiable information collected online about an individual consumer that can be used to locate or to contact that individual directly. When you download and use this Application, Unum collects some information that is personal, as well as some non-personal information that would not, on its own, identify an individual person. When non-personal information is combined with other information so that it does identify an individual person, we treat that combination as personal information.

Unum collects personal information through this Application when you or someone else acting on your behalf affirmatively provides it to us. HOW WE SHARE THE PERSONAL INFORMATION WE COLLECT

We share your personal information as needed to service your insurance policy and administer your leave requests or as otherwise permitted or required by law. This could include sharing information with our "affiliates" (companies within the Unum corporate family). Subject to applicable law, we also may share information with: people and entities who perform insurance, business, and professional services for us, such as helping us pay claims and detect fraud; medical providers for insurance and treatment purposes; insurance support organizations; group policyholders for reporting and auditing purposes; parties to a proposed or final sale of insurance business or study purposes. We also may share personal information without your consent when otherwise required or permitted by law, such as in response to a subpoena, or an inquiry from a government or law enforcement official. In the event of a merger, acquisition, reorganization, bankruptcy or similar event, information collected through this Application may be transferred to or assigned to our successor, if permitted by law and done in accordance with applicable law, without your consent.

Unum does not share nonpublic personal health information about you to market any products or services to you.

Unum does not share nonpublic personal financial information about you with companies that want to sell you products and services that are not related to insurance or other financial services. For example, we do not share your information with advertisers, third-party advertising networks, or telemarketers.

SAFEGUARDING INFORMATION

We have reasonable physical, electronic, and procedural safeguards to help us protect the confidentiality and security of your information that we process through this Application. However, no method of transmitting or storing information via the Internet and mobile platforms can be guaranteed to be 100% secure. As a result, although we strive to protect your personal information, we cannot guarantee the security of personal information transmitted through this Application and therefore the use of the Application is at your own risk.

We limit access to personal information to authorized employees and contracted parties who need the information to provide insurance products or services to you, or to operate, develop or improve our Application.

The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Application, we ask that you not share your password with anyone. If you believe that the security of your password has been compromised or that, for some other reason, your interaction with us via this Application is no longer secure, please notify us of the problem immediately by contacting us at privacy@unum.com.

CALIFORNIA DO NOT TRACK DISCLOSURES

Currently, we do not respond to browser "do not track" signals.

RETENTION AND DELETION OF PERSONAL INFORMATION

We will retain your personal information for as long as necessary to provide you with the Application, or as needed to comply with our legal obligations or to enforce our agreements. If you want to request a deletion of your personal information, please email your request to privacy@unum.com. Note that we need to retain certain records for legal or internal business reasons. Some of your information may remain on backup systems, in compliance with applicable law.

CHILDREN

We do not use the Application to knowingly solicit data from or market to children under the age of 13. If a parent or guardian becomes aware that his or her child has provided us with information without their consent, he or she should contact us at privacy@unum.com. We will delete such information from our files within a reasonable time.

UPDATING YOUR PERSONAL INFORMATION

In certain cases, you may have the ability to view or edit your personal information that we collect through this Application. In the event personal information you provided through this Application is no longer available online and you wish to obtain a copy of it, or if you become aware that the information is incorrect and you are unable to correct it using our online or other customer services, please contact us for assistance at: Chief Privacy Officer, Unum, 2211 Congress Street, C476, Portland, ME 04122, or at privacy@unum.com, and include your full name, address, telephone number, and insurance policy number (if we have issued a policy).

THIRD PARTY TERMS AND CONDITIONS

Please note that your access to and use of the Application may be subject to the third party terms and conditions and privacy notices of application stores and mobile software platforms. You recognize and agree that Unum is not liable for any such third party terms and conditions and their use of your personal information.

GOVERNING LAW

This Privacy Notice is governed by the laws of the State of Tennessee, without giving effect to principles of conflict of laws.
