Who will be able to assist me in the process?

  • A local representative is ready to answer any questions you may have about your voluntary benefits coverage.
  • A personal billing coordinator will be assigned as your single point of contact and is available to take your calls daily.

What type of service can my employees expect from Unum?

Your employees can expect great service through our Customer Contact Center.

  • Representatives in the Customer Contact Center are available for your employees Monday through Friday, 8:00 AM to 8:00 PM Eastern Time. Our toll free number is 800-635-5597.
  • Representatives can assist you with policy change selections as noted below.

What if my employees want to make a change to their policy?

  • Representatives can assist you with policy changes that include:
  1. Name
  2. Surrenders
  3. Beneficiary
  4. Address
  5. Decreases in face amounts
  6. Deletion of riders
  7. Duplicate policy request
  8. Ownership
  • Employees can call our Customer Contact Center to request a customer service form. The form can also be accessed online.
  • Customer Service forms will only be processed when all of the necessary information is complete. Incomplete forms will be returned to the employee for missing information.
  • Completed forms can be faxed to 423-642-5055 or mailed to:

    Unum will make the change within five (5) business days.

Can employees take their policy with them should they leave the company?

  • Yes, employees own their personal voluntary benefits coverage. Employees can keep the coverage, regardless of work status, as long as they continue to pay current premiums due to their existing policies.
  • List bill (payroll) employees can be placed on home direct bill for the following reasons:
  1. Employee is out on leave of absence or has left your place of employment.
  2. Non-payment of premiums for two consecutive payroll deductions.
  • We recommend utilizing our convenient bank draft method in paying monthly premiums. Our Customer Contact Center can provide your employee with a form to be used in the collection of their bank information.
  • Other home direct bill options are also available for your employees' selection. These options include quarterly, semi-annual or annual billing directly to the employee's home address.
