The Provident Agency, Inc. (PAI) currently holds an agent agreement to represent the Unum Group insurance subsidiary companies (Unum). PAI only offers Unum products for Group Life and Disability insurance coverages. Unum has authorized PAI to market, underwrite, sell, administer premium billing, commissions, and claims, and provide customer service support on Unum's Group Life and Disability products. PAI has exclusive rights to market, sell, and administer Unum's First Responder Accident & Health (A&H) product. Under the terms of the agent agreement, Unum compensates PAI via Base and Supplemental Commissions for sales and administrative services. Unless directed otherwise by the customer, standard base commissions are calculated as a percentage of premium per the products listed below.

Supplemental commissions are calculated as a measure of net premium growth (new business and premium increase) on the PAI block of business. Compensation terms are evaluated annually and subject to change.
